Adjusted thirty-day readmission was lower in the intervention arm across all three participating hospitals, with the largest difference among patients discharged over a weekend (Okafor et al., 2024), as illustrated in Fig. 2a. The effect size is consistent with the pooled estimate reported for nurse-led discharge planning in comparable cohorts (Brenner et al., 2025), as shown in Fig. 3b-e, with subgroup results analysed across all study sites.
